Memorial Occupational Health at Decatur Memorial Hospital works with area employers to improve and maintain employee health. 

Highly skilled staff utilize state-of-the art technology when caring for patients. On-site services include: audiograms, drug testing, fingerprinting and background checks, vaccinations and respirator fit testing.

We focus on injury care and injury prevention to help employers maintain a healthy work force.

Physicals

  • Bus driver physicals
  • Bus drivers may obtain the required physical examination through Memorial Occupational Health.

    Please review the DOT Exam Readiness Questionnaire, which may answer common questions and provide information regarding any documentation you need to bring with you to the exam. While bus physicals are not DOT physicals, there are areas that defer to DOT? with regard to injury or illness. See DOT Exam Readiness Questionnaire on the resource page.

  • Department of Transportations (DOT) exams
  • Our Certified Medical Examiners perform DOT exams to determine if a person is medically fit for duty, based on national criteria. Please review the DOT Exam Readiness Questionnaire, which may answer common questions and provide information regarding any documentation you need to bring with you to the exam.   • Environmental exposure exams
  • Environmental exposure exams currently include those for asbestos and silica. Other environmental exposures can be considered. Please call 217-876-4900 to discuss.

  • Fit for Duty/ Return to Work exams
  • Our providers perform Fit for Duty exams to determine if an employee is physically and psychologically able to perform their current job in a safe manner. These exams are completed for workers if a non-work related injury or illness has occurred in order to determine job fitness or if accommodations are needed. They may also be completed for workers who want to return to work after recovering from an injury.

  • Hazmat/Hazwoper physicals
  • Hazmat/ Hazwoper physicals are completed by our providers for workers exposed to hazardous chemicals or materials that could cause health problems. Examination requirements vary based on possible exposure.

  • Independent Medical Evaluations (IME)
  • An Independent Medical Evaluation (IME) is the evaluation of an injured party, which utilizes an independent medical professional to issue an unbiased opinion on an injury. Our physician is able to perform these exams as an independent third party. These exams do not include treatment or medical advice but provide an employer or individual with an evaluation.

  • Pre-placement physicals
  • Memorial Occupational Health provides pre-placement physicals, drug screenings, vaccinations and titers.

  • Worker compensation injuries
  • We offer work injury care and provide a Worker’s Compensation Claims Specialist to help guide you through the process.

Clinical Testing

  • Audiograms: We travel to your location and use our state-of-the-art KUDU wave technology onsite. We can perform up to 10 audiograms at a time.
  • EKG: we provide this testing within our facility.
  • Pulmonary function test
  • TB testing: one step, two step and Quantiferon gold
  • Vision testing
  • X-ray (in house): our in-house X-ray allows us to expedite your appointment without the delay of traveling to the hospital for imaging.

Other Services

  • Confined space training facility: our confined space training facility is available for groups to rent.
  • Education/onboarding meeting space: our clinic has a large meeting room available to rent that is fully equipped with audiovisual capabilities. It can support up to 24 employees comfortably and can be fully tailored to meet your company’s needs.
  • Ergonomic assessments/ job site analysis: our physical therapists are trained to assess work environments and offer recommendations to reduce risk of workplace injuries. A written report with detailed ergonomic solutions will be provided.
  • Fingerprint/ background check: this service is available for concealed carry, teaching and healthcare professions. These can be completed at our facility, or onsite at your location.
  • Foreign travel: Memorial Occupational Health offers a foreign travel clinic. We work with you and your employees to determine what is required based on travel destination and medical needs. This service is also available to community members. Please contact our facility for specific needs.
  • Lift evaluations (human performance evaluations/HEPs): lift evaluations may be utilized as part of a pre-placement physical or as an adjunct to a fit for duty exam. Tasks performed are based on employee’s job description and their specific job demands. 
  • Workplace program consultations: consultations provided through this clinic help to establish or refresh your company’s workplace program to help maintain compliance with regulatory bodies or general guidance. We help you foster a safe and compliant workplace.
